1. The service

MedCoord is provided by MEDCOORD. It helps care coordinators, clinics and patients keep a patient's medical documents, history, medication and appointments in one place and prepare summaries for physicians. By using MedCoord you accept these terms; if you use it on behalf of an organisation, you accept them for that organisation.

2. Not a medical device and not medical advice

MedCoord is a tool for organising information. It is not a medical device within the meaning of Regulation (EU) 2017/745. It does not diagnose, does not recommend or adjust treatment and does not monitor patients in real time.

Summaries, consistency checks, reminders, translations and the answers of the portal assistant, including everything produced with artificial intelligence, can be incomplete or wrong. They must be checked against the original documents and never replace the judgement of a physician. Decisions about diagnosis and treatment belong to the treating physician.

3. Not for emergencies

Do not use MedCoord in an emergency. Messages sent through the portal are not read around the clock. If you think your life or someone else's life is in danger, call 112 (or your local emergency number) immediately.

4. Accounts and access

Coordinator accounts are created by invitation and sign in with a one-time code sent by email. Patients enter the portal with the access code received from their coordinator. Keep codes and links confidential, and tell your coordinator or write to cosmina.palade@gmail.com at once if you believe someone else has used them.

A link for a physician gives read-only access to anyone who holds it, until it expires or is revoked. Send it only to the physician it is meant for.

5. Your responsibilities when you add patient data

• You add data only about patients you are entitled to represent or care for, on a valid legal basis (for example the patient's consent or a care relationship).
• You check the AI proposals before saving them and keep the file accurate.
• You do not upload unlawful content or malicious software, do not try to reach files that were not shared with you, and do not probe or disrupt the service.

6. Availability and changes

We work to keep MedCoord available and secure, but we do not guarantee uninterrupted operation. Keep the original medical documents: MedCoord is not the archive of record. We may change or discontinue features, and we give reasonable notice of changes that matter.

7. Liability

To the extent permitted by law, MEDCOORD is not liable for indirect damage or for decisions taken on the basis of information shown in MedCoord without checking it against the original medical documents. Nothing in these terms limits liability that cannot be limited by law, including towards consumers.

8. Personal data

How personal data is processed is described in the Privacy policy. Organisations that use MedCoord for their patients sign a data processing agreement with MEDCOORD.

9. Intellectual property

The software, the brand and the design belong to MEDCOORD. The content you add remains yours or your organisation's.

10. Ending the use

You can stop using MedCoord at any time and ask for your account and data to be deleted (see “Account and data deletion”). We may suspend an account that breaches these terms or endangers the security of the service.

11. Governing law and contact

These terms are governed by Romanian law. Disputes that cannot be settled amicably are brought before the competent courts in Romania; consumers keep the protection of the mandatory rules of the country where they live. Questions: cosmina.palade@gmail.com.
